How it is all started...
Our Vision & Founding
Located in Delta, BC, Crescent Health was founded on a simple yet ambitious vision: to create a modern, patient-centered clinic where family medicine and specialty care live seamlessly under one roof. Initiated and founded by Dr. Mohammad Hurmat Ali and executed in close collaboration with operations manager Syed Mohsen Rizvi, the clinic officially opened its doors on June 1, 2026. Today, we are proud to deliver integrated, compassionate care to individuals and families across Delta, Surrey, and surrounding communities.
Multidisciplinary Care & Procedures
We offer a growing range of services including family medicine, pediatrics, gynaecology, psychiatry, internal medicine (with special interest in Cardiology), mental health counselling.
Community & On-Site Pharmacy
To support seamless continuity and convenience, patients have access to an on-site pharmacy right alongside their medical care. As we grow, Crescent Health remains deeply committed to expanding our clinical capabilities to provide coordinated, high-quality, and compassionate healthcare tailored to our community's needs.
Services
Multidisciplinary Healthcare
Crescent Health offers a growing range of medical and mental health services, including Family Medicine, Pediatrics, Gynecology, Psychiatry, Internal Medicine, Cardiology-focused assessment and preventative care, Mental Health Counselling, and Minor Surgery and Office-based Procedures.
Our internal medicine services include a particular interest in cardiovascular health. Under the direct supervision of internist Dr. Robert Shaw, Crescent Health is expanding its cardiac assessment capabilities to include preventative and diagnostic investigations such as exercise stress testing (ETT), Holter monitoring, and ambulatory blood pressure monitoring (ABPM).
Minor Surgery and Procedures
Crescent Health has a dedicated minor surgery and procedure room designed to provide selected procedures in a convenient outpatient setting. Services include circumcision, assessment and treatment of lumps and bumps, incision and drainage procedures, and other appropriate minor procedures.
On-Site Pharmacy
An on-site pharmacy further supports continuity and convenience by allowing patients to access pharmacy services at the same location as their medical care.
